Se presenta el caso de un hombre de 60 a\u00f1os, fumador cr\u00f3nico, que se someti\u00f3 a una tomograf\u00eda computarizada (TC) tor\u00e1cica debido a un dolor tor\u00e1cico no espec\u00edfico, el cual revel\u00f3 un n\u00f3dulo pulmonar en el l\u00f3bulo superior derecho. La biopsia confirm\u00f3 c\u00e1ncer de pulm\u00f3n de c\u00e9lulas no peque\u00f1as (NSCLC, por sus siglas en ingl\u00e9s), estadio I. El paciente, sin s\u00edntomas respiratorios notables, fue tratado con resecci\u00f3n quir\u00fargica del tumor. La tomograf\u00eda y los estudios de ganglios linf\u00e1ticos no mostraron met\u00e1stasis, por lo que se opt\u00f3 por un tratamiento quir\u00fargico. Posteriormente, se le inici\u00f3 terapia adyuvante con quimioterapia basada en platino. El pron\u00f3stico es favorable dada la detecci\u00f3n temprana y la ausencia de enfermedad metast\u00e1sica. Se estableci\u00f3 un seguimiento con tomograf\u00eda computarizada cada tres meses para evaluar posibles recidivas. Este caso destaca la importancia de la detecci\u00f3n incidental de c\u00e1ncer de pulm\u00f3n, un hallazgo cada vez m\u00e1s frecuente debido al uso de t\u00e9cnicas de imagen en la pr\u00e1ctica cl\u00ednica diaria.<\/p>\n

We present the case of a 60-year-old man, a chronic smoker, who underwent a thoracic computed tomography (CT) scan due to non-specific chest pain, which revealed a pulmonary nodule in the right upper lobe. The biopsy confirmed stage I non-small cell lung cancer (NSCLC). The patient, with no notable respiratory symptoms, was treated with surgical resection of the tumour. CT and lymph node studies showed no metastases, so surgical treatment was chosen. Subsequently, adjuvant therapy with platinum-based chemotherapy was started. The prognosis is favourable given the early detection and absence of metastatic disease. Follow-up with CT scan every three months was established to assess for recurrence. This case highlights the importance of incidental detection of lung cancer, an increasingly frequent finding due to the use of imaging techniques in daily clinical practice.<\/p>\n

Caso Cl\u00ednico:<\/strong><\/p>\n

 <\/p>\n

Motivo de consulta<\/strong>:
\nPaciente masculino de 60 a\u00f1os, con antecedentes de tabaquismo cr\u00f3nico (30 paquetes\/a\u00f1o), consulta por dolor tor\u00e1cico de 3 semanas de duraci\u00f3n. El dolor es leve, no tiene relaci\u00f3n con la respiraci\u00f3n ni con actividad f\u00edsica. Niega tos, disnea, hemoptisis o p\u00e9rdida de peso.<\/p>\n

 <\/p>\n

Historia cl\u00ednica<\/strong>:
\nEl paciente tiene antecedentes de hipertensi\u00f3n y diabetes tipo 2, ambos controlados. No tiene antecedentes familiares de c\u00e1ncer. En la exploraci\u00f3n f\u00edsica no se encontraron signos relevantes, aunque se observ\u00f3 un leve dolor a la palpaci\u00f3n en la regi\u00f3n tor\u00e1cica derecha sin masas palpables. Debido a la persistencia del dolor tor\u00e1cico, se solicit\u00f3 una tomograf\u00eda computarizada (TC) de t\u00f3rax para descartar patolog\u00edas pulmonares.<\/p>\n

 <\/p>\n

Pruebas diagn\u00f3sticas<\/strong>:
\nLa TC de t\u00f3rax revel\u00f3 un n\u00f3dulo pulmonar en el l\u00f3bulo superior derecho de 2.5 cm sin evidencias de adenopat\u00edas mediast\u00ednicas ni pleurales. Debido a la sospecha de malignidad, se realiz\u00f3 una biopsia por punci\u00f3n, que confirm\u00f3 c\u00e1ncer de pulm\u00f3n de c\u00e9lulas no peque\u00f1as (NSCLC), con caracter\u00edsticas histol\u00f3gicas de carcinoma escamoso. Los estudios adicionales, incluidos la tomograf\u00eda por emisi\u00f3n de positrones (PET-CT), no mostraron met\u00e1stasis en ganglios linf\u00e1ticos o en otros \u00f3rganos, y la evaluaci\u00f3n de la funci\u00f3n respiratoria fue normal.<\/p>\n

Tratamiento<\/strong>:
\nSe opt\u00f3 por cirug\u00eda de resecci\u00f3n pulmonar mediante lobectom\u00eda derecha, con m\u00e1rgenes quir\u00fargicos libres de tumor. Dado el estadio I del c\u00e1ncer, se decidi\u00f3 iniciar un tratamiento adyuvante con quimioterapia basada en platino (cisplatino y gemcitabina) para minimizar el riesgo de recurrencia.<\/p>\n

 <\/p>\n

Pron\u00f3stico<\/strong>:
\nEl pron\u00f3stico es favorable debido a que el c\u00e1ncer fue diagnosticado en una etapa temprana (estadio I) y no existen met\u00e1stasis. La intervenci\u00f3n quir\u00fargica y el tratamiento adyuvante aumentan significativamente las probabilidades de supervivencia a largo plazo. Sin embargo, se requiere un seguimiento estrecho para detectar posibles recidivas.<\/p>\n

 <\/p>\n

Seguimiento<\/strong>:
\nEl paciente fue citado para seguimiento cada tres meses, con tomograf\u00eda computarizada de t\u00f3rax para detectar recurrencias locales o met\u00e1stasis a distancia. Adem\u00e1s, se realizar\u00e1n controles cl\u00ednicos regulares para monitorizar la funci\u00f3n pulmonar y los posibles efectos secundarios de la quimioterapia.<\/p>\n

 <\/p>\n

Discusi\u00f3n<\/strong>:
\nEl c\u00e1ncer de pulm\u00f3n de c\u00e9lulas no peque\u00f1as (NSCLC) es la forma m\u00e1s com\u00fan de c\u00e1ncer pulmonar y, a menudo, se diagnostica en etapas avanzadas. Sin embargo, en este caso, el tumor fue detectado incidentalmente mediante una tomograf\u00eda computarizada realizada por otro motivo. Este hallazgo resalta la importancia de la imagenolog\u00eda en la pr\u00e1ctica cl\u00ednica diaria y la relevancia de la detecci\u00f3n temprana en el pron\u00f3stico de los pacientes. El tratamiento quir\u00fargico en combinaci\u00f3n con quimioterapia adyuvante es el enfoque est\u00e1ndar para los tumores en estadio I, y las tasas de supervivencia a 5 a\u00f1os son elevadas en este grupo de pacientes. La vigilancia postoperatoria es crucial para detectar recurrencias, ya que, aunque el tratamiento inicial puede ser curativo, el riesgo de recidiva sigue siendo una preocupaci\u00f3n.<\/p>\n
