Hipotiroidismo en el embarazo

Monitorización del hipotiroidismo materno

Varios investigadores han sugerido que los niveles de la hormona estimulante de la tiroides (TSH) deben ser monitorizados para evaluar el reemplazo de la hormona tiroidea durante el embarazo 25, 26, 27. Otros autores han sugerido los intervalos de tiempo en los que se debe medir los niveles de TSH: a las 6-8 semanas, a las 16-20 semanas y a las 28-32 semanas 4. Por otro lado, otros autores han descrito que los niveles séricos de tiroxina (T4) deben también ser monitorizados ya que esta hormona es capaz de atravesar la placenta durante el embarazo 28. A pesar de la importancia de la monitorización de la TSH y T4, en la actualidad no existen rangos de referencia claros para los niveles séricos de T4 y TSH en el embarazo 26. Por este motivo los actuales protocolos recomiendan que el ginecólogo establezca los niveles séricos ideales de T4 y TSH durante el embarazo 26.

Conclusión El tratamiento del hipotiroidismo materno es esencial ya que los resultados adversos tanto para la madre como para el bebé se reducen considerablemente, si no se eliminan, cuando los pacientes son tratados. Incluso cuando el tratamiento se inicia tarde en el embarazo, los bebés de las madres tratadas mostrarán un mejor desarrollo neurológico que los bebés de las madres no tratadas.

Los profesionales sanitarios deben promover el uso de fármacos con el fin de lograr un nivel de TSH y T4 adecuado en el embarazo. También se recomienda el seguimiento neurológico de los bebés de las madres hipotiroideas con el fin de identificar las deficiencias cognitivas lo más pronto posible y proporcionar un manejo adecuado.
