Cuidados de la urostomía

En los últimos años, el cáncer de vejiga ha sido una patología grave ya que ha ido aumentando hasta ser la segunda neoplasia urológica con más frecuencia.

RESUMEN:

En los últimos años, el cáncer de vejiga ha sido una patología grave ya que ha ido aumentando hasta ser la segunda neoplasia urológica con más frecuencia. Una vez invadidas las capas musculares la práctica más común más común es la cistectomía radical, en la cual se extirpan la vejiga y también los tejidos que tiene a su alrededor. En la mujer los órganos reproductivos y en el hombre, en la mayoría de casos, exéresis de la próstata y vesículas seminales. Siempre que haya, por el motivo que sea, una alteración que no deje que el sistema urinario funciones de forma normal es preciso realizar una ostomía urológica o urostomía, en la cual la orina que se produce en el riñón se desvía, evitando así, su paso por la vejiga y dándole salida al exterior a través del estoma con la intención de mantener la funcionalidad del riñón.

Dependiendo del tipo de derivación de la orina nos encontramos con dos tipos de ostomías: ureterostomía y nefrostomía.

NEFROSTOMIA: daría salida a la orina hacia el exterior mediante un catéter introducido a través de la piel (costado) directamente al riñón afectado

URETEROSTOMIA: se le daría salida a la orina desde los riñones al exterior a través del propio uréter

La ureterohileostomía tipo bricker es el método más utilizado como derivación permanente y que menos complicaciones presenta. Consiste en aislar una parte del ileon creando un conducto (ileal) en el que abocarán los dos uréteres. En esta derivación el estoma eliminará mocosidad y orina, por la parte de intestino añadida

INTRODUCCION:

Al principio el paciente es cuidado en la cama ya que será el propio personal de enfermería el encargado de ello, puesto que las primeras veces puede producir dolor e incluso fatiga al ser un proceso desconocido para el paciente. Una vez el paciente urostomizado esté preparado se le irá enseñando poco a poco para que el mismo, en un futuro, de pie en el baño, frente a un espejo, sea quién lo pueda hacer, llegando a ser una rutina diaria totalmente normal para él.

Pequeños consejos para la higiene:

  • Nos lavaremos bien siempre las manos antes de cualquier manipulación, ya que una correcta higiene en los pacientes urostomizados es de vital importancia. Por ello debemos tener en cuenta que es recomendable cambiar la bolsa por la mañana antes de ingerir ningún líquido o al menos hayan transcurrido unas 3 horas de la última ingesta.
  • A la hora de proceder al aseo es preferible ducharse antes que bañarse
  • Cuando nos dispongamos a lavar el estoma habrá que hacerlo con agua y jabón neutro, sin frotar y secar después la zona periestomal suavemente con pequeños toquecitos evitando frotar
  • Tener recortado el disco a medida que ajusta el estoma, para así  proteger la piel periestomal. Quitar el papel protector y pegar el dispositivo alrededor del estoma siempre de abajo a arriba. Si fuera necesario, recortar el vello que pudiera haber alrededor del estoma. Es de gran importancia que el disco quede perfectamente ajustado al estoma ya que será el encargado de sostener, posteriormente, la bolsa que recogerá la orina expulsada
  • Evitar utilizar cremas, para así facilitar una mejor adhesión del disco a la piel
  • Los discos deben de ser manejables, seguros y discretos.

OBJETIVO:

Buscaremos la convivencia del paciente en su día a día con la urostomía mediante la reeducación de su higiene personal (limpieza del estoma, vaciado de bolsa, etc…) y la implantación de unos hábitos saludables  como son una nutrición adecuada y evitando por completo el consumo de tabaco y alcohol ya que pueden afectar negativamente en el efecto de los medicamentos. Es recomendable beber líquidos (agua, infusiones…) de forma abundante evitando el exceso de consumo de café, té y bebidas gaseosas. Debemos evitar el estreñimiento incluyendo en nuestra dieta abundante fruta y verdura y alimentos ricos en fibra. Además será necesario seguir estrictamente los controles médicos que se nos indiquen.

UROSTOMY CARE

SUMMARY:

In recent years, bladder cancer has been a serious pathology since it has been increasing to be the second most frequent urological neoplasm. Once the muscle layers are invaded, the most common practice is radical cystectomy, in which the bladder and surrounding tissues are removed. In women, the reproductive organs and in men, in most cases, excision of the prostate and seminal vesicles. Whenever there is, for whatever reason, an alteration that does not allow the urinary system to function normally, it is necessary to perform a urological ostomy or urostomy, in which the urine produced in the kidney is diverted, thus avoiding its It passes through the bladder and exiting it through the stoma with the intention of maintaining the functionality of the kidney.

Depending on the type of urine bypass we find two types of ostomies: ureterostomy and nephrostomy.

NEFROSTOMY: it would expel the urine outwards through a catheter inserted through the skin (side) directly to the affected kidney

URETEROSTOMY: urine would be expelled from the kidneys to the outside through the ureter itself

Bricker-like ureterohileostomy is the most commonly used method as a permanent shunt and has fewer complications. INTRODUCTION:

At the beginning, the patient is taken care of in bed since the nursing staff will be in charge of it, since the first few times it can cause pain and even fatigue as it is an unknown process for the patient. Once the urostomized patient is prepared, he will be taught little by little so that he, in the future, standing in the bathroom, in front of a mirror, who can do it, becoming a completely normal daily routine for him .

Small tips for hygiene:

– We will always wash our hands well before any manipulation, since proper hygiene in urostomized patients is of vital importance. Therefore, we must bear in mind that it is advisable to change the bag in the morning before ingesting any liquid or at least 3 hours after the last intake.

– When proceeding to the toilet it is preferable to shower before bathing

– When we are ready to wash the stoma we will have to do it with water and neutral soap, without rubbing and then drying the peristomal area gently with small touches avoiding rubbing

– Have the disc trimmed as the stoma adjusts, in order to protect the peristomal skin. Remove the protective paper and stick the device around the stoma always from the bottom to the top. If necessary, trim the hair that might be around the stoma. It is of great importance that the disc is perfectly adjusted to the stoma since it will be responsible for holding, later, the bag that will collect the ejected urine

– Avoid using creams, to facilitate better adhesion of the disc to the skin

– The discs must be manageable, safe and discreet.

OBJECTIVE:

We will seek the coexistence of the patient in his day to day with the urostomy by re-educating his personal hygiene (stoma cleansing, emptying of the bag, etc …) and the implementation of healthy habits such as proper nutrition and completely avoiding consumption of tobacco and alcohol as they can negatively affect the effect of medications. It is advisable to drink liquids (water, infusions …) abundantly avoiding excessive consumption of coffee, tea and soft drinks. We must avoid constipation including in our teeth abundant fruit and vegetables and foods rich in fiber. It will also be necessary to strictly follow the medical controls indicated to us.
